SPLASH Registration Withdrawal Form

To withdraw from SPLASH lessons at SC-Carmel, please submit this online form at least seven (7) prior to the start of the session.

**Please submit the form just one time. Once processed, we will email you a confirmation email within a few days letting you know it’s all taken care of.**

If you are having a scheduling conflict, we would be happy to help you switch to another day and time or address any other issues you may be having.

If you are not swimming at SCSC, please keep your child active in swimming so they don’t lose their skills! For their safety and your peace of mind, all children need to be able to swim a strong freestyle stroke with good breathing. Most experts agree that the goal of 300 yards of freestyle is a good measure of water safety. And even for children who can swim, please be a designated Water Watcher when any children in your care are in or around the water. Their safety depends on your constant parental vigilance!

I understand that withdrawal forms must be received at least seven (7) days prior to the start of the sessions. Cancellations received less than seven (7) days before the first day of lessons will only be eligible for a 50% refund. Any cancellations made on or after the first day of lessons will not receive a refund. Withdrawals are final. If you change your mind, you must re-enroll.
